- Died of a service-connected disability,
- Died on active duty and the dependent are not otherwise eligible for the department of defense TRICARE benefits.

How to Apply for CHAMPVA
If you provide all of the optional and required documents, and all your application is complete, then it takes approximately up to six weeks after they get your package. Until then, you get your CHAMPVA identification card and related materials. below are the materials needed to apply.
