  • The housing status (whether traditional, private, public, employer-provided or otherwise)
  • Applicant’s signature and date
  • Authorizing another person to help you apply and get your benefits
  • Authorized person’s information (phone number, address)
  • Domestic violence occurrence
  • Information on people in the household if applying for a household (names, gender, birth date, social security number, and relationship to you).

  • Presence of children under 18 years of age. Specify their names.
  • Presence of borders in the household.
  • Foster care payments for anyone in the household. Specify the person.
  • Specify if anyone in the household is a resident or intending to leave the state.
  • Citizenship/ nationality of those in the household.
  • Specify the mental and physical disabilities of anyone in the household.
  • Specify the working status of everyone in the household.
  • Any child support court order or legal obligation on anyone in the household.
  • Specify if anyone in the household has a child or any adult-dependent care expenses.
  • Specify if there is any health insurance you have, anyone in the household that is 60 years of age and above has or any disabled person in the household has.
  • Any kind of health insurance.
  • Specify any out-of-pocket medical expenses.
  • Kind of housing expenses.
  • Utility expenses
  • Specify cash in the bank for you or every other person in the household.
  • Specify life, burial, or prepaid funeral insurance.
  • Mention any buildings, land real estate or burial plots you or any other person in the household may bathe.
  • Specify any pension or retirement account for you or anyone in the household.

After giving out this information, there is a penalty warning at the end of the form. This warns you about the punishment that you could incur if you give out any false information or hide any important information from the agency in other to get the benefits quickly from the program.

The amount of benefit received from the program is usually based on the size of the household. The number of people in the household would determine the amount of benefits that would be sent out to the household. There is a maximum amount of benefits sent out.

In regards to this, the eligibility of an applicant or household is also dependent on the income earned by that household. There is a specific limited amount of income that should be earned by the household. Exceeding this limit may render an applicant or applicant ineligible.

The household’s income is calculated monthly or yearly before the deduction of tax. Then the gross income is obtained after calculating the federal poverty level. This gross income of the household is compared to the maximum income allowed by the program against the number of households. If it exceeds, the household may not be eligible to receive benefits.

The maximum income allowed by the program per household size is;

  • For a family of 1, the gross maximum income allowed is $1,354
  • For a family of 2, the gross maximum income allowed is $1,832
  • In a family of 3, the gross maximum income allowed is $2,311
  • For a family of 4, the gross maximum income allowed is $2,790
  • For a family of 5, the gross maximum income allowed is $3,269
  • In a family of 6, the gross maximum income allowed is $3,748
  • For a family of 7, the gross maximum income allowed is $4,227
  • For a family of 8, the gross maximum income allowed is $4,708
  • In a family of 9, the gross maximum income allowed is $5,184

  • For a family of 10, the gross maximum income allowed is $5,663
  • For a family of 11, the gross maximum income allowed is $6,142
  • In a family of 12, the gross maximum income allowed is $6,621
  • For a family of 13, the gross maximum income allowed is $7,100
  • For a family of 14, the gross maximum income allowed is $7,579
  • In a family of 15, the gross maximum income allowed is $8,058

And for each additional member, an amount of $479 is added to the gross maximum income level.
